These Terms apply to all visitors, parents, legal guardians, referring physicians, discharge planners, social workers, and any other individual or organization who accesses the Site. If you are using the Site on behalf of a minor child, you represent that you are the child’s parent or legal guardian, or otherwise have legal authority to act on the child’s behalf.
HomeLogic Healthcare provides in-home pediatric health care services for medically fragile children across Georgia, including skilled nursing care, personal care assistance, therapeutic services, and respite care, delivered under a physician-approved plan of care. Services may be funded through Georgia Medicaid, the Georgia Pediatric Program (GAPP), private insurance, or self-pay arrangements, subject to eligibility and authorization.The Site itself is an informational and intake tool. It allows visitors to learn about our services, coverage areas, and eligibility; submit a referral or inquiry; request or book an in-home nursing assessment or appointment; and contact our care coordination team. The Site does not provide medical care. Care is only provided in person, by licensed clinical staff, under a physician approved plan of care, after intake and eligibility have been confirmed.
Nothing on the Site including service descriptions, condition information, case examples, or blog content constitutes medical advice, diagnosis, or treatment, and no provider-patient relationship is created by browsing the Site or submitting an inquiry form.
If your child is experiencing a medical emergency, go to the nearest emergency room immediately. Do not use this Site, our contact form, or email to seek emergency assistance. Response times stated on the Site (for example, response within two hours, or one hour for after-hours on-call situations) apply to non-emergency care coordination only and are not guaranteed in every circumstance.
Enrollment in services is subject to a clinical assessment, physician orders, and, where applicable, Georgia Medicaid or insurance authorization. Submitting a referral or contact form does not guarantee acceptance into our program, coverage by Medicaid or insurance, or a specific start date for services. Estimated timelines described on the Site, for example same-day assessments or five to ten business day authorizations, are general estimates based on typical cases and may vary based on individual circumstances, payer requirements, or staffing availability. All clinical services are provided pursuant to a physician-approved plan of care and applicable Georgia licensing requirements, separate from these Terms and governed by our clinical intake and consent documentation.
Any health information you submit through the Site’s contact or referral forms is used solely to evaluate and coordinate potential care and is handled in accordance with our Privacy Policy and, where applicable, the Health Insurance Portability and Accountability Act (HIPAA).
