We use the information we collect to respond to inquiries and referrals and evaluate eligibility for services; to coordinate a free in-home nursing assessment and connect you with a care coordinator; to communicate with physicians, hospital discharge planners, or Georgia Medicaid on your behalf, with appropriate authorization, to arrange care; to schedule and confirm appointments; to operate, maintain, and improve the Site; to comply with applicable licensing, Medicaid, and legal requirements; and to communicate with you about our services, unless you opt out of non-essential communications.

3. Health Information HIPAA

Information about a child’s medical condition submitted through the Site is treated as sensitive and is used only to evaluate and coordinate potential care. Once a child is accepted for services, health information becomes part of the clinical record and is protected under HIPAA and governed by our HIPAA Notice of Privacy Practices, which is provided separately at intake and describes your rights regarding protected health information, including access, amendment, and accounting of disclosures.
